Efectividad del método concept global d’épaule en comparación con el shoulder symptom modification procedure combinado con un método de tratamiento convencional en pacientes de 35 a 55 años que sufren de patología no perforante y no calcificante del manguito de los rotadores. Propuesta de intervención.

Abstract

Introducción: El dolor de hombro es la tercera sintomatología álgica del sistema músculo esquelético después del dolor de espalda y dolor cervical. Está considerado como el “mal del siglo XXI”. La tendinopatía del manguito de los rotadores es la lesión la más frecuente y suele inducir largos periodos de baja laboral además de unos costes económicos considerables. Objetivo: El objetivo de esta intervención era demostrar la superioridad terapéutica del método Concept Global d’Épaule (CGE) en comparación con la herramienta diagnóstica Shoulder Symptom Modification Procedure (SSMP) combinada con un método de tratamiento convencional en pacientes que sufren de patología no perforante y no calcificante del manguito. Diseño: Estudio piloto unicéntrico, simple ciego de casos consecutivos con intención de tratar. Tipo TRT durante un periodo de intervención de 12 semanas. Seguimiento longitudinal y prospectivo. Metodología: Se estudió el efecto de ambos tratamientos en relación al dolor, la discapacidad, la fuerza muscular, al control motor y ROM articular. El grupo experimental recibió el protocolo CGE y el grupo control el protocolo SSMP combinado con tratamiento convencional. Resultados: Se espera corroborar la hipótesis inicial y demostrar la superioridad terapéutica del método CGE aplicado al grupo experimental. Discusión: Las variables de confusión podrán influir en los resultados de ambos grupos, o influir más en uno que en el otro según la distribución en los grupos. Las diferentes particularidades de los dos métodos del estudio generan que algunas variables puedan tener superioridad técnica en un tratamiento respecto al otro, y viceversa.

Background: Shoulder pain is the third algic symptomatology of the musculoskeletal system after back pain and cervical pain. It is considered the "XXIst century’s pain". Rotator cuff tendinopathy is the most frequent injury and usually induces long periods of an inability to continue work which has a direct impact on financial income. Objective: The objective of this intervention was to demonstrate the therapeutic superiority of the Concept Global d'Épaule (CGE) method compared to the Shoulder Symptom Modification Procedure (SSMP) diagnostic tool combined with a conventional treatment method in patients suffering from non-perforating and noncalcificating pathology of the rotator cuff. Design: Single-blind pilot study of TRT-type with consecutive cases with intention to treat during a 12-week intervention period. Single-centre reliability study, with longitudinal and prospective follow-up. Methods: The effect of both treatments in relation to pain, disability, muscle strength, motor control and joint range of motion were studied. The experimental group received the CGE protocol and the control group the SSMP protocol combined with conventional treatment. Results: It is hoped to corroborate the initial hypothesis and thus demonstrate the therapeutic superiority of the CGE method applied to the experimental group of this intervention. Discussion: The confounding variables may influence the results in both groups, or influence one more than the other depending on the distribution in the groups. The different peculiarities of the two study methods train that some variables may have technical superiority in one treatment over the other, and that for other variables the opposite occurs.
